Cigarette smoking is banned in public restaurants and in many bars throughout major cities. While there is no federal ban on smoking, some states operate a smoking ban and bars and restaurants have autonomy on many of their public areas with regard to smoking. As an advanced country in terms of medical research, the dangers of long-term smoking are highlighted and it is discouraged in public areas for the safety of the general public. Health care facilities are largely owned and operated by private sector businesses. 58% of US community hospitals are non-profit, 21% are government owned, and 21% are for-profit. According to the World Health Organization (WHO), the United States spent more on health care per capita ($8,608), and more on health care as percentage of its GDP (17.2%), than any other nation in 2011. 64.3% of which was paid for by the government in 2013. Of 17 high-income countries studied by the National Institutes of Health in 2013, the United States was at or near the top in infant mortality, heart and lung disease, sexually transmitted infections, adolescent pregnancies, injuries, homicides, and rates of disability.
A 2014 study by the private American foundation The Commonwealth Fund found that although the U.S. health care system is the most expensive in the world, it ranks last on most dimensions of performance when compared with Australia, Canada, France, Germany, New Zealand, the Netherlands, Norway, Sweden
